Clinic Nineteen long COVID specialists will see any adult or paediatric patient suffering from ongoing symptoms following acute COVID infection or vaccination. We welcome indefinite referrals.

Instructions for downloading the PDF referral
- In order for the PDFs to remain writable please access them via Google Chrome or Adobe Acrobat
- Click 'Download'
- Select a location on hard drive (which you can find later, such as the desktop) and save.
- Fill in the referral form digitally and save.
- Email the referral to us by clicking here

- Download the RTF document
- Open a New Letter and from Templates menu select Import Template (if you can't see this, review your permissions)
- Locate downloaded file and open
- From File menu select Save as. Type an appropriate Template name and click on Save.

- Download the RTF document
- Click on Tools > Clinical WP Setup > WP Templates
- File > Import Template
- Find the file where you saved it and highlight it and then click Open
- Enter the template name into the Description. Click on OK.
Medical Director
- Download the RTF document
- Open Medical Director and then LetterWriter
- File menu > Select Modify Template > Select Black template
- File > Select Import - locate downloaded file and click on Open
- File > Select Save as template - type in an appropriate name and click on Save
